For families · The move into care

Mom's moving into care. Here's the checklist.

The weeks before a parent moves into a care home go fast. This is the list families tell us they wish they'd had: what to gather, who to call, and what the home will ask for, in the order you'll need it.

Ten things to have ready

  1. A current medication list

Every prescription, over-the-counter medicine and supplement, with the dose, the time of day and the prescribing doctor. Ask the pharmacy for a printout to check it against.

  2. Insurance cards, front and back

Medicare, any supplement or Advantage plan, Part D, long-term care insurance and any private policy. Photograph both sides of each.

  3. Doctors and providers

Primary doctor, specialists, dentist, pharmacy: names, practices and phone numbers in one list.

  4. Power of attorney

For finances and for healthcare. If there isn't one yet, start now; it can take longer than the move itself.

  5. Advance directive and wishes

A living will, any do-not-resuscitate order, and a short note of what matters most to your parent.

  6. Recent medical history

Diagnoses, allergies, recent hospital stays and anything the care team should watch for.

  7. Emergency contacts

Who the home should call first, second and third, and who is allowed to make decisions.

  8. The home's admission paperwork

Ask early for the full packet so nothing waits on a missing signature.

  9. The family plan

Who visits when, who handles bills, and who the home should talk to. Agree it before the first week, not during it.

  10. The comforts of home

Photos, a favourite blanket, labelled clothes and glasses, hearing aids and chargers. Small things make the first nights easier.

Keep it in one place, not a folder of photocopies.
